TextCare Pro Care Coordinator

About the Role

The TextCare Pro Care Coordinator will support TextCare Pro patients in ensuring they have exceptional care and an exceptional experience. Working for and with a team od providers, they will manage responsibilities like managing authorizations and forms, coordinating with pharmacies and specialists, driving referrals and diagnostic follow-through, supporting patient adherence, and closing loops across labs, imaging, and remote patient monitoring workflows.

TextCare Specialists and MAs will handle all inbound care requests, and the TextCare Pro Care Coordinator will focus on patients already in the care process.

Success in this role means moving patient care forward independently, competently, and vigorously.

Essential Functions

  • Assist with scheduling and guiding patients through basic processes

  • Fill out forms (e.g., prior authorizations) as fully as possible

  • Coordinate with specialist practices

  • Support TextCare Pro workflow by ensuring required information is obtained for client-specific tasks (e.g., biometrics for wellness reviews)

  • Send/Fax outbound orders and manage outbound documentation

  • Track and follow up on outstanding labs & imaging orders, including abnormal labs

  • Manage referrals, diagnostic orders (e.g., sleep studies, Cologuard), scheduling barriers, and records retrieval

  • Provide ongoing, condition-specific patient education to support adherence and close care gaps

  • Execute patient engagement workflows (lab reminders, RPM reminders, abnormal lab outreach, and Retool-triggered follow-up), and general outreach to improve adherence

  • Monitor population health analytics flags (e.g., BP log noncompliance) and initiate follow-up message with PR approval

  • Perform basic pharmacy troubleshooting (e.g., resolve medication availability issues, clarify insurance barriers through direct pharmacy communication)

  • Identify and connect patients with appropriate assistance programs/resources

QUALIFICATIONS/EXPERIENCE

  • 4+ years of work experience, preferably in customer service or healthcare

  • CCMA (or similar clinical accreditation) strongly preferred

  • Highschool diploma or equivalent required
