Position: Palliative & Hospice RN
Monogram Health is seeking a skilled Registered Nurse, with experience in palliative care, to join the team due to continued nationwide growth. The Palliative & Hospice RN supports chronically ill patients through advanced illness management, palliative care management, and hospice coordination. The ideal candidate will have a deep understanding of palliative care, excellent communication skills, and the ability to coordinate patient care with empathy and a strong attention to detail.

This role is a key member of the palliative care team and enables palliative-focused physicians and APPs as well as our market care teams to provide the highest impact care to our patients. This role promotes patient-centered care by ensuring seamless communication between care teams, patients, and families.

Roles & Responsibilities
Palliative Care Triage & Clinical Support
- Triage urgent palliative care needs and escalate appropriately.
- Provide clinical support to regional palliative physicians, including documentation and workflow assistance.
- Assist with conservative management support for select patients.
- Participate in co-visits and telephonic follow-ups as needed.
Hospice Enablement & Coordination
- Facilitate hospice referrals and coordinate informational visits.
- Provide education to patients and families considering hospice.
- Identify patients enrolled in hospice by external providers (e.g., PCPs).
- Maintain regular communication with hospice agencies to receive updates on referred patients, ensuring continuity of care and timely information sharing through the patient’s hospice journey.
- Deliver education and workflow support to market teams, including travel for training and quality improvement.
- Provide regular updates and hospice-related reporting to regional palliative physicians, market palliative specialists, and MPEs.
Panel Management & Patient Education
- Conduct palliative-focused patient care tasks and “tuck-in” calls in collaboration with the patient’s care team.
- Engage high-need, hard-to-reach patients to encourage participation in palliative care.
- Support scheduling and pre-visit education for complex patients.
- Attend co-visits and coordinate telephonic follow-ups with providers.
Advance Care Planning & Documentation
- Obtain ACP documentation from external offices and hospitals.
- Assist with lower-complexity ACP discussions and patient education and ensure accurate documentation.
- Collaborate with care teams to ensure ACP preferences are reflected in care plans.
Position Requirements
- Graduate of an accredited School of Nursing
- Currently licensed as a Registered Nurse
- 2+ years of experience working in palliative-focused care, geriatric home-based care, and/or hospice care
- Ability to travel to assigned market(s) up to 20%
- Self-starter with the ability to work independently and collaboratively in a remote environment with minimal supervision
- Ability to show empathy and quickly build relationships with patients and physicians
- Familiarity with Microsoft Office and web-based applications
- Excellent verbal and written communication skills
- Prior experience in value-based care models preferred
