Job Description

A bit about this role:

This position is an amazing opportunity for an ambitious nurse practitioner to take on a new leadership role in the Transitions of Care (TOC) Program and to help develop the clinical excellence of the team. The TOC APP Manager will be responsible for the clinical and operational oversight of the APPs in the ever-growing TOC program, while continuing to provide clinical care to patients. The position will join the TOC management team (Medical Director, APP Managers, Operations Leads, Nurse Director & Managers, and Staff Supervisors) and together work to strengthen the program’s collaborative culture, empower the front line staff to achieve clinical excellence, and surface opportunities to improve patient care. This position will report into the Medical Director of the TOC program.

Required skills and experience:

  • 5+ years working in clinical practice as an Advanced Practice Provider (NP/PA).

  • At least 1 year of experience seeing post-discharge patients as an Advanced Practice Provider (NP/PA).

  • 2+ years of previous management or team leadership experience.

  • An active and clear RN and APRN license or Physician Assistant license in your home state, as well as national APRN certification and/or the ability to get licensed in multiple states is required at time of hire and must be maintained while employed at Devoted Medical.

  • Able to work a Monday through Friday schedule.

  • Experience performing visits over telehealth video platforms and managing post-transitions patients and being able to identify the need for immediate interventions.

  • Experience in managing acute/chronic disease exacerbations including CHF exacerbations, diabetic emergencies, COPD exacerbations and hypertensive emergencies.

  • Experience collaborating on process improvement projects with members outside of your role.

  • Willingness to grow/change using PDSA principles.

Your Responsibilities and Impact will include:

40% of the time will be dedicated to providing TOC patient care. Time spent on clinical responsibilities will fluctuate depending on the number of direct reports.

  • Provide excellent care to patients in the 30-day post discharge period, identifying and filling gaps in care, escalating symptomatic patients through appropriate pathways while managing as indicated, and reducing preventable readmissions

60% of the time will be dedicated to serving as APP manager and part of the TOC leadership team. Time spent on managerial responsibilities will fluctuate depending on the number of direct reports. These responsibilities include:

  • Oversight and accountability for the clinical performance of the APP organization in the TOC program

  • Performance management of front line TOC APPs to ensure the team is meeting required targets

  • Develop, track, and manage clinical and operational KPIs for the TOC APPs

  • Identify areas for improvement and implement strategies to achieve targeted metrics

  • Manage, empower, inspire, and align the ever-growing team of TOC APPs, to include bi-weekly one on one meetings with each direct report

  • Set personalized goals for direct reports and review individual performance

  • Collaborate closely with program medical director and APP managers to enhance team processes and workflows within the APP team and across TOC roles

  • Assist with recruitment strategy, hiring, and onboarding of new TOC APPs

  • Develop clinical processes and policies for the program

  • Monitor clinical resource allocation and execute changes to meet the visit needs of the practice

  • Schedule oversight to include management of time off and requests associated with time off, monitoring daily, weekly, and monthly schedule density, and all schedule changes impacting patients scheduling.

  • Oversight of on boarding guidance for new providers in clinical and process development in conjunction with the education and training team

  • Serve as the liaison to TOC leadership on status, issues, and areas of opportunity in the APP organization

Salary range: $140,000 - $155,000 annually

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
