A bit about this role:
As a Care Coordinator at Devoted Health, you will play a vital role in supporting our care coordination function. You will advocate for our members, assisting nurse case managers and ensuring that individuals have the resources and support necessary to achieve better health outcomes.
In this position, you will help connect members with their healthcare needs, including scheduling appointments and securing durable medical equipment, through proactive outreach and follow-up communication. We are looking for a caring, compassionate, and solution-oriented individual who is enthusiastic about delivering an outstanding experience for Devoted Health members.

Your Responsibilities and Impact will include:
Conduct health risk assessments through calls to members to understand their health and needs fully.
Coordinate with provider offices and home health agencies to ensure members receive necessary support.
Check in with members over the phone to assess their well-being and confirm they have received the assistance they need.
Actively participate in interdisciplinary rounds as an essential member of the care team.
Review care plans for members to identify additional ways to provide support.
Key player in the care plan operations workflows for our SNP members.
Collaborate closely with other Devoted teams to address member needs effectively.
Provide operational and administrative support to complex care department
Required Skills and Experience:
Minimum education: High school diploma or GED equivalent
2+ years of experience in healthcare administration or home care
Desired Skills and Experience:
Proficient in technology (experience with Gmail/G Suite)
Comfortable working in an interdisciplinary team environment
Strong organizational skills and attention to detail
Previous experience in social services
Fully bilingual (English/Spanish)
Salary Range: $25 - $30 / hour
Our ranges are purposefully broad to allow for growth within the role over time. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
